Why Blood Pressure Is Still Measured in mmHg

A blood pressure reading of 120 over 80 is one of the most quoted measurements in all of medicine, yet few people could name its unit. It is millimeters of mercury, written mmHg — a unit that literally describes how many millimeters a column of liquid mercury would rise inside a glass tube when pushed by your blood’s pressure. Long after mercury instruments left most clinics, the unit they created remains the worldwide language of blood pressure.

It is a striking survival. Medicine has modernized nearly everything else, but this nineteenth-century unit refuses to retire, and the reasons are more sensible than mere habit.

What the mercury column measured

Early blood pressure instruments, called sphygmomanometers, connected an inflatable arm cuff to a vertical tube of mercury. Inflating the cuff squeezed the artery shut; as air was released, the pressure at which blood began pulsing through again pushed the mercury to a certain height — the systolic reading. The height where flow ran smoothly again gave the diastolic. Mercury was chosen because it is extremely dense, about 13.6 times denser than water; a water-based gauge would need a tube roughly 1.6 meters taller to show the same pressures.

The units behind the numbers

One mmHg is defined as about 133.3 pascals, and 760 mmHg equals one standard atmosphere. So a systolic pressure of 120 mmHg is roughly 16 kPa — about 16 percent above ambient pressure, measured relative to the atmosphere around you. Some countries’ laboratories report blood gases in kPa, and the SI system would prefer everything in pascals, but blood pressure has stayed stubbornly in mmHg nearly everywhere.

Why medicine never switched

The practical case for keeping mmHg is strong. A century of research, treatment thresholds, and clinical intuition is written in the unit: clinicians instantly recognize 140 over 90 as a hypertension boundary and 90 systolic as worryingly low. Converting to kilopascals — 18.7 over 12.0 — would shrink the familiar numbers into awkward decimals and invite dangerous confusion during the transition, with no diagnostic benefit whatsoever. Regulators have generally granted mmHg a permanent exemption for medical use, and modern digital monitors simply display the old unit electronically. Related measures like eye pressure and intracranial pressure stayed loyal to mmHg for the same reason.

When you next read 120 over 80, picture the pressure of your blood, riding on top of the atmosphere, hoisting a pencil-thin column of mercury twelve centimeters up a glass tube — the instrument is gone, but its ruler lives on.
